Central Community Hospital Skilled Care
Mission: Every Patient Matters
Skilled Rehabilitation is the term used for an extension of hospitalization for those who are being discharged from their hospital stay, however are not quite ready to go home. Patients qualify for skilled care when they need nursing services, or extended therapies. The care provided is generally covered by insurance.
There are many reasons why a patient may need skilled rehabilitation. It is our goal to accommodate those needs. We set an individualized care plan for you or your family member. We strive to meet personal goals. For many, that means having the ability to go back to living at home.
Our skilled care unit is open and staffed with physicians, nurses and ancillary care givers.Patients have the opportunity to receive treatment and therapy needed to transition back to home safely, while being able to be close to home to enjoy the comfort of family and friends.
