Cardiac Rehabilitation and Pulmonary Rehabilitation Department

Cardiac rehabilitation is a heart-monitored program and pulmonary rehabilitation is a heart and lung monitored program. Both include education, exercise and encouragement to help change behaviors that affect each condition. Central Community Hospital (CCH) is committed to providing high quality, personalized health care to patients close to home. CCH works collaboratively with patients and their providers in the surrounding communities to achieve excellence in health care. 

Cardiac Rehabilitation Program

  • Candidates for this program include patients who have experienced the following: heart attack, open heart surgery, stent placement, heart valve repair/replacement, heart transplant, or chest pain that is controlled by medication.
  • The patients referred to the Cardiac Rehabilitation program will follow a 6-8-week treatment plan. The patient’s treatment plan will consist of one-hour rehabilitation sessions three times each week. The patient will be supervised by a nurse and/or respiratory therapist that are certified in Advanced Cardiac Life Support (ACLS).
  • Ordering providers are kept informed of their patient’s progress in the program.

Pulmonary Rehabilitation Program

  • Candidates for this program include patients who have experienced the following: reoccurring asthma, bronchitis or emphysema, partial lung removal or transplant, lung diseases caused by exposure to dust, molds and/or chemicals.
  • The patients referred to the Pulmonary Rehabilitation program will follow a 6-8-week treatment plan. The patient’s treatment plan will consist of one-hour rehabilitation sessions three times each week. The patient will be supervised by a nurse and/or respiratory therapist that are certified in Advanced Cardiac Life Support (ACLS).
  • Ordering providers are kept informed of their patient’s progress in the program.
